The Day Services Support Staff assist in implementing the service plan for each consumer through direct training and coaching.
As a member of the Day Services team, helps achieve person-centered goals for each individual served, which may include, but is not limited to, recreational and social activities, personal care, daily living skills, communication skills, and community integration activities.
The Day Services Support Staff supervise consumers to ensure their health and safety in a manner that increases the individual's independence and participation in community activities as appropriate for each consumer.
Essential Job Functions:
1.
Participates as an active member of a person-centered team that identifies and provides training and support services to each consumer.
2.
Assists with the development and implementation of structured activities for consumers in areas of training in daily living skills, community integration, recreation and socialization.
3.
Teaches consumers how to perform specific tasks with increased independence through formal and informal instructional strategies.
4.
Facilitates the consumer's learning of appropriate attitudes and behaviors when on the mobile work crew or volunteering or partaking in leisure activities; promotes proper peer interaction and acceptance of responsibility and supervision.
5.
Provides supervision and support for consumers at volunteer sites and when enjoying community activities.
6.
Assists consumers in learning community integration skills including socialization, grooming, communication and accessing community services.
7.
Maintains accurate records and reports, including electronic case records, at the day services for assigned consumers.
8.
Identifies and responds to the individual needs of consumers including physical, social and personal needs.
9.
After completing DDD Medication Training, administers medications in accordance with Enable's policies and procedures which conform to DDD guidelines.
Records administered medications.
10.
Actively engages consumers throughout the work day including a working lunch.
11.
Responsibly becomes educated on consumer preferences and individual needs by reviewing the consumer's IHP and through regular one on one conversation.
12.
When directed, implements behavior modification plans for specific consumers.
13.
Informs manager, Nurse and Behaviorist when significant behavioral or medical changes occur in consumers.
14.
Participates in team, agency staff meetings and required in service training sessions, including DDD Pre-Service Trainings.
Completes online training classes as assigned.
15.
Oversees mobile work crew activities.
Provides guidance, supervision and training to consumers in mobile work crew so they may effectively complete assigned duties.
16.
Provides safe transportation for consumers between group homes and day services, to any volunteer or activity sites and to any destinations as directed by supervisor.
17.
Cooperates with Enable, Inc.
and the Division of Developmental Disabilities during any inspection or investigation.
18.
Successfully completes and demonstrates proficiency in all areas of required training.
19.
Acknowledges the employee's name will be checked against the Central Registry of Offenders Against Individuals with Developmental Disabilities.
20.
Completes other tasks as assigned by Day Services Manager.
